OVERVIEW:

We are looking for a Part Time Registered Nurse (20 hrs. NOC) to join our team.

 

As a member of the Interdisciplinary Team, the RN Case Manager is an experienced Registered Nurse who provides skilled palliative and end of life care in the community setting. The RN Case Manager is responsible for promoting the Hospice philosophy. This individual facilitates the development of a collaborative interdisciplinary plan and ensures the overall coordination, collaboration and medical management of hospice services for patients and their significant others.

YOU HAVE:

  • Completed a professional nursing program accredited by the National League for Nursing (prefer completion of a baccalaureate program in nursing).
  • One year experience in acute care facility or community health agency, (hospice experience a plus).
  • Current California RN Licensure and CPR Certification
  • Valid CA driver’s license, auto insurance, reliable transportation and a willingness to use personal vehicle in the course of employment.
  • Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 50 lbs.)
  • Availability to work night shifts.
