We are looking for a Full Time Intake Assistant to join our team in Patient Access.


Reporting to the Clinical Manager of Patient Access, this position is responsible for taking all hospice information calls regarding the intake of new referrals and obtains pertinent information to facilitate an evaluation for hospice service. This position is also responsible for the timely and accurate entry of a variety of data including referral data, monthly reports, meeting minutes and other data entry as directed.  This position also performs a variety of clerical and administrative tasks, which may include coordinating meetings, interviews, organizing applications, copying documents, faxing, filing, answering telephones, assisting manager in tracking mandatory agency requirements and special projects by performing the following duties.



  • High school diploma or general education degree (GED), two years college preferred.; with two years’ experience in health care data entry, preferably in hospice care information systems. 
  • Able to function autonomously with flexibility and confidence and have the ability to function in an interdisciplinary team setting.
  • Must possess good time management skills and demonstrate excellent communication skills and the ability to deal with difficult and complex situations.
  • Demonstrated ability to function independently and with minimal supervision.
  • Possess the ability to establish and maintain positive working relationships with both Administrative and clinical staff.
  • Proficiency in Microsoft and Google programs required.
  • Demonstrated ability to work with health team personnel.
  • Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 10 lbs.)
  • 4 – 10 hour shifts, ability to rotate weekends/holidays
