Job Description Summary
The Nurse Practitioner provides holistic hospice and palliative care within their scope of practice, integrating palliative interventions into a population-focused model of care. This role supports field-based services and includes completing hospice admissions as needed, typically one to two times per week.

Essential Job Functions and Responsibilities
- Provide timely, effective patient care with close coordination alongside RNs and LVNs. Care includes in-person visits, phone communication, and secure messaging. Visit frequency and duration are based on clinical acuity and triage assessments.
- Manage ongoing symptoms for assigned patients, including supervision of nursing staff and appropriate use of pharmacologic and non-pharmacologic interventions
- Apply nursing process and clinical judgment in patient care
- Evaluate and approve prescriptions, including controlled substances, as clinically indicated
- Deliver a holistic approach aligned with hospice philosophy, addressing physical, emotional, spiritual, and psychosocial needs
- Consult with hospice Medical Director or other physicians as needed
- Attend and contribute to scheduled interdisciplinary group (IDG) meetings
- Conduct timely face-to-face visits for assigned patients and present findings at IDG
- Review hospice eligibility for referrals when requested by intake, marketing, or clinical leadership
- Participate in monthly provider meetings
Position Requirements
- Active California RN and Nurse Practitioner licenses, including furnishing license
- Current DEA registration and NPI (assistance available if needed)
- Valid California driver’s license and reliable, insured vehicle
- Current CPR/AED certification
- National NP certification through AANP or ANCC
- Proficient in electronic medical records and documentation
Preferred Qualifications
- Hospice experience strongly preferred
- Certification in hospice and palliative care (CHPN or ACHPN), or willingness to obtain
- Experience in field-based medical care settings such as mobile care or outreach teams
