Grief Specialst and YoloCares for Kids Coordinator

Provides grief support services to YoloCares’ and community clients and coordinates the YoloCares for Kids programs/services with developmentally appropriate support for children and teens. Offers classes and workshops on grief-related topics. Supports the work of Center for Loss & Hope.

Essential Duties and Responsibilities

• Coordinates registration, attendance, and follow-through for YoloCares for Kids (YCKids) programs, and partners with other YoloCares staff to promote programs.
• Provides short-term individual telephone, and in-office anticipatory and grief support to the children and teens of YoloCares patients and clients.
• Participates in the review and amendment of on-going Plans of Care for individuals enrolled in YCKids individual programs.
• Provides short-term, developmentally appropriate anticipatory loss and grief groups for YoloCares adult clients and the wider community.
• Makes referrals to community resources as appropriate.
• Consults with the Center Manager regarding current complex cases to assess client’s needs and develop and implement support and/or follow a Plan of Care.
• Documents all interventions according to YoloCares policies and procedures and meets required time frames for their completion.
• Trains and provides on-going support to YCKids volunteers.
• In consultation with the Center Manager, offers classes, workshops, and presentations on grief-related topics. 
• Participates in performance improvement activities related to processes and outcomes of the Center for Loss and Hope services, as requested by the Center Manager.
• In consultation with the Center Manager, develops new grief support programs and services as needed, and assists with grant funding deliverables. 
• With other team members helps maintain our EMR, implement our CLH Global Plan of Care, and ensure compliance with policies/procedures, and Joint Commission standards. 
• Performs other specific assignments as assigned.

• Master’s degree in counseling, social work, or spiritual care; certification in grief support theory and practice; or a resume showing equivalent experience.
• Ability to communicate well in English (spoken and written) is required. Proficiency in Spanish is welcomed.
• Ability to assess grief severity, self-harm risk, and appropriate client grief interventions required.
• Strong computer skills, including Excel, Word, and Outlook. 
• Experience and skills in facilitating grief support groups.
• Compliance with agency’s mandated reporting policy.
• Ability to successfully meet agency background screening requirements.
• Strong knowledge and skills in the provision of emotional support for children and youth who are experiencing acute and/or complex grief or emotional crisis.
• Experience and skills in the provision of grief support groups.

• Capacity to facilitate regular team meetings to promote collaboration, share resources, and address challenges. 
• Ability to conduct periodic reviews of case files and service plans to ensure compliance and quality of care.
• Willingness to train in trauma informed care, cultural competence, evidence-based practices for care management, and/or healthcare best practices. 
• Ability to learn and understand ECM and CS services and CalAIM initiatives. 
• Proficiency in data management and reporting tools.
• Able to hear, speak, read, write, and type well enough to perform required job duties.
• Able to lift and carry up to 15 pounds.
• If driving on YoloCares business, must have a valid California driver’s license and state-required automobile insurance.
• Must be willing and able to flex hours to meet occasional appropriate business needs during evenings or weekends.
